Check that adb and fastboot can detect the device and you gave granted permission for the connection on the device. Then if all that is ok try the installer again and if you have nothing on the OPO to keep use the wipe option on the installer.

Please check that both adb and fastboot can detect your device before continuing. Also please confirm which version of the installer you are using. Not totally sure from the screenshot. If you are using the new 0.5.3 beta and adb is working read the other post in this section and report the bugs etc.
As your on Windows 15 sec adb from xda developers may help.
