        I tried to install ubuntu touch on my Oneplus One with the installer and at the point that I had to restart to the Recovery I got stuck on the Powered by Ubuntu screen,
        QHSUSB_BULK is not showing up in Device Manager.

          @mmvanheusden what OS is on your pc/laptop? If you have another phone handy use that to check ADB and fastboot are working correctly if not remove and reinstall. If you got through fastboot recovery should be installed if it boots clear cache using android actions and try again. Also don't forget it could be a cable issue try a few others.
          If you are using windows install 15 sec ADB from XDA developers and install system wide and remove all other phone drivers.

            Was Android > 5.1 installed before? You might need to go back to stock Android from an older verison to get the cache partition right.

              If i try to boot in recovery i get the same screen: powered by Ubuntu
              And if i connect the phone to my laptop,
              device manager wont refresh
              I use Windows 8.1.
              And 15 seconds adb

                @mmvanheusden Get the latest installer from here https://github.com/ubports/ubports-installer first. Remove adb and fastboot and phone drivers. Restart pc, and reinstall adb system wide and phone drivers. Connect your device in command prompt type fastboot devices check your device is seen then the same for adb. You will now know that is working or not. Run the installer again with the wipe option ticked if you can.
                If fastboot works you will have recovery, boot to that (power and volume down buttons held together) when asked if it all runs through great if push files fails clear cache etc with android and ubuntu actions then try again.
                If that fails look at Florians answer above or try running Ubuntu 16.04 on a live usb stick.

                  i have tried it on zorin os but the device is not detected by adb or fastboot.

                          @mmvanheusden Then I think yours may well be a partion size issue if non of the above work. As @Flohack suggested you will probably need to reflash android first before trying again.

                            if I try to reboot to Recovery then I also get the "powered by ubuntu screen" so how can I install stock android if I can't flash twrp via fastboot

                              @mmvanheusden If you have powered by ubuntu screen fastboot will have worked at some point (try another cable they are often an issue). See if it will go to fastboot power and volume up held together.

                                      I had similar issues with the OnePlus One.

                                      I think your log is indicating that USB debugging was not set up:

                                      "device unauthorized.\nThis adb server's $ADB_VENDOR_KEYS is not set"

                                      This is a bit tricky to get around as it means only Fastboot can be used so you'll need to get into that mode.

                                      In this respect I found that the phone buttons keys had to be held for some time before anything happened per these instructions:

                                      If you can get to Fastboot, to get the phone back to stock and then have another go at setting up USB debugging follow this guide which only uses Fastboot commands:

                                        it does not boot in fastboot no matter how long I keep the buttons pressed.

                                          OK. From a blank screen (Off) try only holding the volume up and then the power button as well until something happens.

                                          Also have you left the phone plugged in as the OnePlus One can misbehave on very low charge.

                                            @mmvanheusden You will get a very small oneplus logo on a black background, fastboot mode is not that visible on screen.
