For further improvements please find below screenshots from Lifeguard ICE app. They show front and back end of the app. It allows the user to note what they want (privacy) and have all the information at the same screen, the additional information is displayed by swiping the middle section only.
Hope this will help us getting a better idea and inspiration to complete the app.

It's quite a lot of screenshot and frankly I have no idea to make thumbnails so I made a quick collage of both front and back end screenshots. -

Wow, this is getting very good.
Can you explain why height, eye colour and hair colour are relevant? I can see why weight is relevant, because it has a correlation with how certain medications should be dosed.
Is it for purposes of identification in extreme cases? -

Hey guys, the screenshots are from the Windows 10 mobile app I used. I am not a developer, so the only way for me to help getting this ICE feature The collages have been posted here to give you an idea and have it available for discussion so that @syper will have a relative good idea for the final version, reducing the time for him to spend on details.
Re hair and eye color, I really wouldn't know other than a way for emergency services and bystanders to identify the one in need for help corresponds with the owner of the phone and his ICE data

Finally I got an answer from NHTSA and here it is:Thank you for proactively reaching out to NHTSA to use the Star of Life symbol. If you choose to use the Star of Life symbol itself, or a close iteration of such, you'll need NHTSA's permission to do so. The guidance for using the symbol is found on www.EMS.gov at: https://www.ems.gov/vgn-ext-templating/ems/sol/HS808721.pdf
If you choose to not use the Star of Life symbol, you will not need to seek our permission.
Unfortunately, the NHTSA Office of EMS is unable endorse one product over another. Since the In Case of Emergency (ICE) program is not one of our programs, we won't be able to create an alternative to the Star of Life symbol.
I hope this helps and please call me at xxx-xxx-xxx if you'd like to discuss this further or proceed to get permission from NHTSA to use the Star of Life symbol.

Great to see the NHTSA has replied and the reply is clear on the do's and don'ts. Thank you again for taking the time and sharing.
What do you propose we use as a logo?
The easiest way imo would be to use the objective/non-copyright alternative SOS logo (directly available from the emoji database).
If we were to do it the right way (read: most commonly known amongst emergency services) we should contact NHTSA, ask for their permission and adopt the exact same Star of Life as depicted in the filed one (with or without serpent, with or without the ICE marking in the Star.
With the document at hand I don't see any point in proposing another design as we would then need to go through NHTSA anyway because of the usage of the 'Star' to make it legit.
If needed the logo/icon can be changed later on.
We should make a decision and hopefully someone (@Syper ?) can continue to work on the ICE feature and get the app to the next level.

@3T_Ed How about keeping the original icon but adding ICE within the white cross. Something like this:
This is just a mockup, the cross should be more to the left. I made the cross a little bigger, just so that the text gets bigger.
